The questionnaire is undated, but was created circa 1951 or 1954, likely for use in either the 1952 “Report on Health Education Major” or 1955 “Report on the Graduates in Health Education.” The 1952 report was authored by Dr. Elena M. Sliepcevich during her tenure at Springfield College, Massachusetts. It is likely she authored the questionnaire and the 1955 report as well.
